Camper Transportation Form
This form must be completed to notify us how your child is getting to and from Camp.
Health Form Part One (To Be Filled Out By Parent Only)
Health Form Part Two (To Be Filled Out By Doctor Only)
These forms are required by the American Camp Association. They must be filled out and received prior to Camp.‘
Part Two’ needs only to be completed and signed by your child’s doctor if:
~ This is their first summer at Camp Twin Creeks
~ A signed Medical Form was not provided last summer
